How to Create a Meta Lead Ad Form and Have it Integrated with Your Gym Lead Machine Account

In this guide, we’ll walk you through how to create a Meta Lead Ad Form using Meta Business Suite and how to integrate it into your Gym Lead Machine account to ensure your leads flow smoothly into your CRM.
Step 1: Open Meta Business Suite
- Go to Meta Business Suite.
- Open an existing campaign or create a new one.
- Select one of your active ads or create a new ad.
Step 2: Create a New Lead Ad Form
- Under the ad setup, locate the section to manage Lead Forms.
- Click Create Form.
You’ll be guided through several steps:
Form Type:
Choose between:
- More Volume – Easier and faster for users to submit (ideal for most gyms).
Higher Intent – Adds a review step to improve lead quality.
Intro & Questions:
- Add a headline or greeting.
- Include qualifying questions (e.g., fitness goals, availability).
Upload a relevant background image for branding.
User Info Fields:
Select the type of information you want to capture:
- Full Name
- Phone Number
Privacy Policy:
Be sure to include a link to your gym's privacy policy.
Completion Message:
Customize your thank-you message and include a call-to-action (like booking a free trial or consult).
Step 3: Adjust Autofill Settings for Better Lead Accuracy
Meta offers an autofill feature that uses the contact info tied to a user’s Facebook account. While convenient, it may result in outdated information.
To ensure accuracy:
- Click Settings in the top-right corner of the form builder.
- Disable the Autofill Contact Info option.
- This will require leads to manually enter their current information.
Step 4: Submit Your Lead Form for Integration
Once your form is published:
- Log into your Gym Lead Machine account.
- In the lower left corner, click on Request Forms.
- Select Lead Ad Integration.
- Fill out the details of the Lead Form you just created.
